New Student Registration

For information regarding enrollment, please contact the Registrar, Mrs. Rodriguez, at prodriguez@ssfusd.org or 650-877-8806 ext. 6012.

 

When registering a student please bring the following documents with you:

- Verification of Residence, one of the following:

  • PG&E bill in guardian/parent's name
  • Residency Statement (obtained from district office)
  • Open Enrollment/ InterDistrict Transfer (district office)

- Guardian/Parent ID (name and address MUST match PG&E bill)

- Current Immunization Record: Guide to Immunizations Required for School Entry  

including:

  • 4 Polio
  • 4 DTP
  • 1 dose TDap after age 7
  • 2 MMR
  • 2 Varicella

- Proof of Age – one of the following: 

  • Certified copy of a birth certificate/record or statement by the local registrar or a county recorder certifying the date of birth 
  • Baptism certificate (duly attested) 
  • Passport 
  • If none of the above documents is obtainable, an SSFUSD Age and Birth Verification Affidavi

- High School Transcript (including withdrawal grades) OR most current 8th grade report card if incoming 9th grader.

-Special Education Documents (IEP, 504 Plan, etc.) if applicable
